The diagnosis of traumatic splenic rupture is generally not difficult. According to the trauma history, clinical manifestations and the results of abdominal puncture, the correct rate is as high as 90%. However, the spleen injury with little blood loss and mild symptoms is easy to be ignored. In addition, the symptoms of multiple organ injury caused by trauma are not easy to distinguish from the spleen rupture. Although the clinical manifestations of spontaneous splenic injury are similar to those of traumatic splenic injury, the diagnosis of spontaneous splenic injury is often not made before surgery due to the absence of obvious trauma history. Therefore, if the condition permits, imaging examinations such as B-ultrasound and CT should be performed for further diagnosis and dynamic observation of the extent and scope of spleen injury. It should be pointed out that these objective indicators are also of great significance for clinical classification, treatment formulation and efficacy evaluation. 1. Laboratory examination: In the case of splenic rupture and hemorrhage, routine blood tests such as red blood cell count and hemoglobin are progressively decreased, while white blood cells may slightly increase. Although other tests such as electrolyte, coagulation function, blood type and amylase are not specific for diagnosis, they should also be used as routine tests for abdominal trauma to help differentiate and diagnose other complicated injuries and judge the condition.
2. Diagnostic peritoneal puncture visual tube and peritoneal lavage: It is an invasive examination with a positive rate of more than 90%, and it is of great help to diagnose whether there is organ damage in the abdominal cavity and what kind of organ damage. If the extracted fluid is fresh, non-clotting or bloody, it proves that the abdominal organs are bleeding. If the extracted fluid is turbidized, it is the characteristic of gastrointestinal rupture.
